DOH ISSUES RED PLACARD TO HI SHACK (772 TXS) IN HONOLULU

HONOLULU — The Hawaiʻi Department of Health (DOH) Food Safety Branch (FSB) issued a red “closed” placard and immediately shut down HI SHACK (772 TXS), located at 2002 Kalākaua Ave. in Honolulu.

The food establishment, operated by HI SHACK CORP., received the red placard on Aug. 11, 2026 and must remain closed until all violations are corrected.

The violations include:

  • Refrigeration not holding Temperature Control for Safety (TCS) foods at or below 41 F as required
  • Handwashing sinks in the mobile unit were not accessible for employee use
  • Hot holding temperatures for TCS foods not being held at 135 F or above as required
  • The mobile unit was not operating in accordance with approved plans and menu.

The DOH is requiring the food establishment to take the following corrective actions before it can reopen:

  • Establishment must ensure that all TCS foods are held at 41 F or below.
  • Handwashing sink must be unobstructed and accessible for handwashing.
  • Hot holding temperatures of TCS foods must be at 135 F or above.
  • Mobile unit must operate in accordance with previously approved plans

The operator was instructed to contact the DOH for a follow-up inspection once all items have been corrected.

The DOH FSB protects and promotes the health of Hawai‘i residents and visitors through education of food industry workers and regulation of food establishments statewide. The branch conducts routine health inspections of food establishments where food products are prepared, manufactured, distributed, or sold.
